Know of any good safety videos?

A while back someone here linked a farm safety video from one of the east coast state gov'ts that was very good. I shared it at my job because it was thought provoking and better than the cheesy acted out training videos often presented to us. I work with construction and heavy trucks so that video was good even if it was farm based. It showed the hazards of equipment. I would like to find another equipment safety based video that's "good". I know there are many places to buy such videos but I think personally sharing makes it more meaningful. Any one know of a good safety video available online?
 
In my experience there are no hazards ''created'' by equipment, it is always the people operating the equipment or working in close proximity to the equipment who either are not competent to use the equipment as intended or are careless, lazy, distracted etc. In over 40 years on offshore drilling rigs I have never seen or heard of any accident, major or minor, that was not caused by human error, it is always the people, even in all cases of equipment failure it can be traced back to the people who were supposed to maintain the equipment, or certify it for use, or ensure it was not used beyond its design limits. The company I work for started focusing on human behavior roughly 20 years ago and the industry followed, the result is total incidents down over 95% and serious injuries and fatalities down even more. When I was a kid my Dad used to say, ''pay attention to what your doing and you will be a mile ahead of the pack because most people don't'' that statement is probably more true 50+ years later than when he made it.

That's where I believe you are wrong. If there were no hazard it would be idiot proof and no one would be injured or killed. A PTO shaft is a hazard. If all the safety procedures are followed to a T then there is little or no risk from that hazard. You have to have a hazard plus human error to have an accident of any type. A set of stairs is a hazard. If someone gets careless and doesn't used the hand rail it can result in an accident. Even a car accident starts with the fact that it, the car is inherently hazardous. Add human error, either in operation or maintenance and you have an accident.
