No, they should go flat side toward the washer.

Probably not that critical on a wheelbarrow though.
 
Interesting. It was done on purpose, but I sure don't know why.

I've done it, and it's not easy. You have to first run the nut down the right way to form threads in the lock ring, then take it off and put it on backwards. In my case I did it to make a polishing wheel arbor out of a 1/2" carriage bolt.
 
Back in the 70s my uncle had one that the bolt went threw and there was a cotter pin on the other side. I learn this buy having to change it with my cousin after his idea. We had to clean the barn cutters at his dads farm and were not happy about it because we had other things we wanted to do so his idea was to take the pitch fork and pop the tire and then tell his dad that we cant do the cutters because the tire needs fix. Well this where we learn that that wasn't a good idea because my uncle looked at it and said well I don't have another one and that one is dry rotten so u boys will just have to use it flat. That sure wasn't any fun then. After we got it all done his dad showed up with a new one he got a while back and said hay boys look what I found and now u guys can change it. Later in life I was told that he knew what happen because he seen my cousin do the tire with the fork and to teach us a new lesson.
