Been meaning to post a followup for posterity but got busy. Here goes. After getting shot down by a number of welders (my job was too small potatoes), I was finally able to get a fabricator to make the arm forks. Came out to $100 total for the two forks. Not bad! See photos for the final product.

However, I must have misunderstood one of those dimensions from Bob. I'm pretty sure what I thought was an INSIDE spacing between the arm forks of 5-3/4" may have actually been an OUTSIDE width. Of course, too big is better than too small everything is still working fine EXCEPT the 6-1/2" axle bolt. With 5-3/4" inside spacing, plus the two 3/8" sides, that comes to a total outside width of 6-1/2", leaving no threads for nut tightening on a 6-1/2" axle bolt. Of course, I should have been able to figure this out on paper before getting the thing made, but I didn't. So I just swapped them out for a 7-1/2" bolt. The OEM tire assemblies were ridiculously overpriced so I went with a pair of 11x4.00-5 tires from Amazon for $50 (link below). These are slightly bigger than the factory tires so I wish I had a little more throat depth, but it still works.
