I was driving down the road yesterday, with 14K pounds on my cow trailer. Hit a little bump, and boom. The trailer slid down the sleeve on the coupler. Luckily, I have a flat bed, or there would have been serious damage. Just readjust the height, and tighten those set bolts as tight as I can? Am I missing something?
 

I would get a good look at the inside of the tube. I adjusted mine a little when I got it and it has been ten years. If it has too much rust scale inside, it could interfere with the friction which is all that holds it.
 
You're lucky that's all that happened. There's a custom hauler here who has a couple of trucks and aluminum trailers. His brother was driving one for him one day and got stuck in a farm yard. They hooked on to him with a FWA tractor and took off. The truck moved,but the trailer didn't. It folded the front of the trailer right up and the hitch was sticking straight out toward the truck.
 

Set it to the height you want and drill a 1/2" or bigger hole thru the tongue and put a long bolt or pin thru the hole.
My 20K tandem dually trailer came with the two clamp bolts and a 5/8" pin going thru the side of the neck.
The inner tube as holes every couple of inches for height adjustments, the clamp bolts keeps the hitch tight.

A grade 8 bolt would probably be strong enough wouldn’t it?
 
I would've thought it would've had a pin in it already. A 5/8 grade 8 bolt would work just fine. As Destroked said, the set screws should just keep the hitch from working.
