don't know how, when or where this happened but it did. I cannot for the life of me remove it. any tips ? thanks.

I have had that happen a couple times. I just threaded a screw into it and pulled on the screw.
 
Did you try to loosen the tension by backing the tension sleeve off? Looks like it may be adjusted too tight, unscrew the sleeve some so it won't happen again.
 
Good morning: In the worst case, replace the coupler.... This thing is not a puzzle that must be solved, you can throw it away. My humble opinion.

thanks fellas. I finally got it out and I found another coupler in my "never throw anything away" drawer.
 
The things you learn on this site. I have had that happen, but never knew that the grip could be adjusted.
