Frost plug popped out

Hi. Had my U running the other day, wasnt hot,
temperature wasnt even above 100 on the gauge. All of a
sudden freeze plug pops out of back side of back head.

Engine has less then 15 hours on it, water pump got new
seals, Im a bit at a loss. Anyone have any guesses as to
what happened? Thanks

Plug wasn't installed properly.
 

I agree, I was told to put them in with a little Permatex around the edge of the opening and tap them in with something a little rounded so they were bowed in a little bit, so when presurized they went tighter before they poped out. I don't remember of any coming back out or leaking.
 

I think I used a piece of shovel handle or something wood. Looks like I rounded the end of it on a grinder. It's still in my tool box from years ago.
