56 john deere 620 rim sliding on wheel

on my 620 whenever i am pulling something under load one wheel sides on the rim. I have tightened all bolts and it still does it. makes a horrible sound. thought the tractor was going to explode until i figured out what was happening. the other side is fine. the clamps seem to be in position. Any suggestions as to why it would do this and any solutions would be appreciated. thanks, Bob
 
There are two nubs I think. I will have to check to see if there is a third. However the clamps are all placed so if you back up with a blade for example the rim will go that way.
