3020 load control shaft

I replaced the load control shaft, bushings and seals on my 3020. After it set for a few weeks I graded my drive way with it and when I was walking back in the house afterward I found one of the caps for the load control shaft in the drive way. I know the caps were in good and the bolt set in the cap correct. What went wrong? Luckily the shaft had not moved out any.
 
Some times it all gets worn to where that will happen,, try reinstalling the old cap with a new bolt.

Some of these bolts have the threads removed at the front so the bolt protrudes into the cap further, of course only needed if the threads don't fit into the hole in the cap.

To remove the threads I have chucked the bolt of choice in the drill press and held a file against the front of the bolt to remove the threads for a short length.
