JD 420 W PTO won't Engage

Have the 5 Speed tranny and the PTO will not engage - I've removed the transmission shifter top plate to see that fork M3874T does not actuate the M3590T PTO Coupling far enough forward on the M4278T power shaft despite being limited by the protrusion on the M3318T engagement lever. I do not feel any detent action as I would expect from the L468T Spring and L467T/405R ball bearing.

Any thoughts on how to get the PTO to operate as it should?

I suppose dismantling the entire back half of the tractor is an option.
 
I guess I am trying to understand what went wrong leading to this condition?

I'm not sure that grinding away the limiter will help because even with the transmission cover plate off (removing the lip that acts as a stop), I can't seem to get it to go, nor can I with the cover on but the handle (and limiter) removed.
 
I'm guessing there is play in the bottom end of the rod which connects the lever up top with the fork inside the transmission. In other words, the rod is turning a little inside the fork and not rotating the fork enough.

Take a look at the diagram in the parts book. I seem to recall the shifter had to come off to get access.

I looked at section 30-4 but unfortunately, access seems very limited as the shaft and gears above it take lots of space...
