NH 565 - twine comes out of the needle

This has happened 3 times this year, twice in the last 500 bales. Going along just fine, and look back to see bales with only one string. Check for cause, and find one twine not through the needle, just trailing along the undeside of the bales. Nothing else appears wrong. Rethread and go again. Any ideas why this is happening?
(9000 poly twine, if that makes any difference)

Twine pulling from the disc won't leave it out of the needle (usually). The most consistent cause of twine getting out of the needle eye is too little twine tension at the twine box.

That happened to me yesterday with my MF 224. opened up the twine bin and there was a plastic twine jacket trying to fit through the hole with the twine. I would look at twine tension and any possible drag on the twine.

I think Bob nailed it. I wish it was only 3 times in 500 bales that I see that.
I'm using sisal in my old Ford and that's a fairly regular occourence when the baler starts getting pushed.
Mine is breaking, mostly because the twine fingers catch the twine on a heavy push... or because the ball simply can't feed it fast enough for what the baler wants.
Also check your twine tension... might be too much.

The twine is either tearing or being pulled out of the twine disk. What does the trailing end look like... neatly cut or torn?

Neatly cut, but frayed some...so probably being pulled out of the twine disk, right? Didn't think I was pushing it much when it happened.
