I assume you mean model number? I dont believe the blades in that era had a model number and they had no serial numbers that I have ever seen. Parts book just call them a 42" push blade.
 
Yes the bracket is super easy to modify if you can weld. Only difference is the brackets where it attaches under the grill.

What about the other end of the sub-frame where it slides onto the rockshaft? There is quite an "upsweep" on the wide frame vs no "upsweep" on a narrow frame.
 
Here is my old 124 with a wide frame blade subframe ,should be able to see the curved back end of the subframe. Other picture is the modification at the front mounting. I used 1/4 x 2. Took maybe 30 minutes total to retrofit.
