Ford Popping out of gear?

My Ford 555 Loader (old, ugly, but usually dependable), has started popping out of gear. Any gear, any speed. Also makes some grinding sound in motion but sounds external to the transmission? I did some searching, could it be the throwout bearing needs greased? Is that a thing on these old loaders? Any other likely culprits? Any advice appreciated.
 
The parts site only shows the 6x2 manual reverser or the 4x4 torque converter auto reverser available on the 555.

Does it have two sticks on top of the trans, or one stick with a forward/reverse handle to the left of the steering column?

Two sticks.
 

I disconnected the clutch linkage to make sure it wasn't dragging and that didn't help. The sound is almost erratic when you put it in neutral and let the clutch out. Like something contacting something else at times.
 
That should be a 6x4 manual reverser. Try moving the main shifter all of the way to the right in the middle of the H pattern past the gate for the right forward and reverse positions and then forward and see if there's a third forward gear.

So the main taller shifter is popping out of gear on its own, correct?

Your problem sounds like something is worn internally, like the rail detentes or the depressions that they ride in on the rails, or both or possibly the main shift plate. The detentes are actually called "shift rail interlock" in my older parts book, but the current parts web site simply calls them a "dowel".
 

Have you checked the oil in the transmission, my 4500 sets a lot, trans got full of water and rusted a bearing, wasn't jumping out off gear but was making a noise
