Typical result of age and use- you need to replace the shift lever or build it up to new specs; perhaps the forks themselves are extremely worn as well. If you need to remove the forks, they come off with the top cover. When replacing the unit, put the transmission and the shifter in reverse gear, it is easier to re-assemble that way- one fork needs to go around and underneath.

In the shifting rails there is a pin that suppose to keep it from going into two gears at once. Check to make sure that the pin is in the rail. I have found some where they were missing.
