Lug bolts issue

Today I moved the haybine off the field and back to the farm. Twenty miles from the farthest field to home. About half way I get a bad noise so I pull over and one wheel bolt is gone and three are loose. I tightened them all down. Replaced a couple of them, I keep extras. Drove a couple miles and the noise is back. They are all loose again. Kept this up until I got home. These are bolts that go thru the wheel and the hub and come out the other side. I am wondering about a nut and lock washer? Still scratching my head as to why this is all of a sudden happening. I changed the tire on that wheel earlier this month. The other wheel is just fine. Any ideas?
 
While this may sound funny check the wheel bearing if it is going bad it can cause vibration which can do that. As for the nuts you can do that and you probably would not need lock washers since the nuts would lock just like when you use 2 nuts on something to keep things from coming apart
 
Once the wheel has come loose and the lug bolts/studs have wallowed out the holes, the wheel is junk. It will NEVER stay tight-EVER! Also the studs, nuts and or the bolts probably are junk too. MArk.
 
At the least the wheel needs to come off and the back side ground off flat where the holes are burred from wallowing loose. It may be ruined if the holes are worn too bad. Or if the center is flat, not a pressed surface, you might have a new set of holes drilled between the worn ones.

Same for the hub, it will need to be inspected, checked for flat, and new studs installed with new nuts.

Even then, the nuts need to be checked several times after it is back on. Prevention is key.
 
If rim gets the lug seats hammered bigger then the taper on the bolt, the bolt will unscrew as the bigger hole works on the smaller taper. How many times have you used your fingers in a circle to run off a nut from a long bolt by moving your fingers like a connecting rod. New rim it is. Jim

Replace the rim, if it's not readily available there are some repair options, they're not cheap, either, though.

if things are not too badly worn, you could grind the taper on the nuts flat a bit so they dont bottom out when tightened. even a drop of blue locktite wont hurt. blue stuff is enough to hold and yet be able to remover when needed.
 
Just for a quick fix put flat washers on the rim side and lock nuts on the back side of the bolts then carry a good impact with you,check and tighten frequently for awhile.
 
Or like me you are so happy to get in the hay field that you never notice the wheel coming loose and the next thing all the bolts come out but two and its breaks a chunk out of the casting. Now I check the wheel on the right side of the haybine regularly.
 
If the holes are worn you can get reinforcing rings for welding on loader wheels or other heavy loaded wheels. Just weld on on and lengthen the bolts. Or you can do as you like with the nuts just tighthen bolt then tighten a nut on the backside it will work like a locking nut set up and be fine also. Big thing is if the holes are worn out.

Many years ago had a wagon wheel partially strip threads in hub out. Forget what had happened. Think I might have had a tire blow out. But those partially striped out thread cleaned them up, got longer lug bolts and put in, tightened OK but did not trust them not to strip out rest of threads. Put a lock nut on back side of hub, NO washers. Things stayed tight for years till got rid of wagon when quit farming. This was before I ever heard of anything like locktight.
