I got my CDL physical today and the doctor gave me my copy to send in and kept a copy on file. Where do I send this copy? I thought I had an address but can't find it.
 
Your doctor should have kept a copy for his files and give you 1 or more copies of the full physical (long form) for yourself. Included in your long form copy should be a wallet size card (sometimes a full page) for you to carry around.

Since you have the right to protect your medical history the only people that need a copy of the long form are............
Your DMV will want one to get your CDL
Your employer will need one for his file if you use your CDL for work.

The wallet card is for you to carry around in case you ever get stopped by the DOT. They are not allowed to insist on seeing the long form.
 
You do not need a D O T physical to get your C D L. You only need the physical when you are driving a vechile that falls under the D O T rules and regulations.
 
While it is in the federal regulations that you need a physical to apply for and get your CDL it is a lot eaiser for me to just link my state web site to prove my point.

I did not have to get a physical when I got my CDL. I do need a physical IF I will be getting paid to drive.
 
i got 3 coppies, i keep one in my truck , gave one to my employer, and put the last one in my 'office' files at home

I beg to differ, at least in NY,, I had to renew my license this jan. on my bday, we have an 8 yr license,, you NEED a physical card to get your cdl REGARDLESS if you do it for a living or not,, they will not give you a class license that is CDL without it,,

I cant drive a truck commercially anymore anyway after a major accident put me out of commission, but it was always nice to at least have the cdl, " it helped me out with LEO a few times, I think".... but I can no longer pass the physical, so no more CDL A,,

they send a form with renewal and all stating the need for it... show me different???
 
Just renewed mine in Illinois (4 Year) and didn't need to show the physical card.
Good thing too cause mines expired BUT I don't drive for work anymore.
 
Missouri is the same way you can have one with out going in for a physical I still have mine and renewed it about 2 years ago with out doing more then having a picture taken. probably broke the camera and of course paying the big fee
 
You carry a copy with your CDL and your employer needs a copy. Mo doesn t require a Medical for a CDL, but you have to have it to drive for hire. My new one is 4 weeks old next Wednesday.
 
Appears that there is some confusion between "getting" and "renewing". It may be a Fed Reg to "get" one, but not to "renew" it.
 
Under the new regs. states have to implement physicals and CDL". in order to keep getting federal funds. The deadline has not arrived yet, therefore not all states have started requiring physicals , BUT they will.
