Wheel came of newly purchased trailer

We recently purchased an 18ft utility trailer to transport our Kubota tractor. We have only used 6 or 7 times before today. Today, while using our trailer, one of the wheels came off. I am including a picture of the wheel and where its mounted on the trailer. There was no warning before the tire came off and everything was riding smoothly. Have any of you seen this? A neighbor said it was because of incompatible lug nuts on the wheel, but the wheel may not have been on tight enough?
 
Did you check lug nut torque after pulling it 50 - 100 miles?? Most trailers I have seen have instructions to do this.
 
Back in the 60's when my dad worked for the JD dealer. They sold a new 3020. After a week the owner came in mad because one front wheel came very loose. The owner of the dealership pulled a 3020 manual out from under the parts counter. Showed the farmer where it said the owner was to check the front wheels after a certain number of hours. Marv then asked the farmer if he read his manual. The farmer through his arms up in the air and left.

I'm guessing the wheels on the trailer were to be checked after so many miles.
 
New mag wheels usually have to be retorqued after 25-50 miles. My new PJ came with instructions to retorque after taking possession of the trailer.
 
A gentleman was telling me last week that he had lost a wheel assembly on his way home from picking up his new 18 ft bumper pull. When he pulled the grease caps he found the cotter keys had not been installed on the made in china axles. His dealer fixed his, but they need to be checked.
