Are these things designed to punch the Rivet out of
a knife without first shearing the head of the rivet
off?

Yes. Used one just like that many times in the ?70s. One side punch?s the rivet out the other side sets the rivet. To use it simply set the punch over the rivet to be remove then use a wrench to screw the punch out. To set the rivet unscrew the center screw then screw the outer screw down until it is tight against the sickle section holding the rivet in place. Then screw the center down until the rivet is set.
 
I'm not certain what they were designed to do. But they absolutely will remove the rivet as it is, until the time you don't line the tool up exactly right, and then the punch will break. The punches are easily replaceable, provided they are available. I think I have three of these tools around somewhere, maybe one has an intact punch. I have moved on to the nut and bolt that replaces the rivet.

Had a similar tool for breaking/setting a motorcycle chain link. Of course I never used the punch to remove the old rivet, just cut through the link with a grinder. But always had to set the new one with the squeezer. Mine would flare out a small depression in the tail end of the rivet.

Yes.. I have found that I grind off the head and then punch it out, it works better... then use it to set the new rivet...Otherwise it broke after about 3 uses.... so... beware and careful... works well for "in the field" repairs so you dont have to pull the bar off the machine...
