JB weld might work. Cures out to a nice smooth surface. Will need to scuff the porcelain around the hole to give it some tooth to grab. It will tend to sink into the hole so place a bit of fiberglass cloth or plastic screen over the hole before application. Problem is IF DOES"NT WORK you've got more of a problem. gobble
 
I'll second the JB weld.

Grind the rust away to clean metal. If close enough to the floor, use some expanding foam to support the back side, whittle a shallow pocket into the foam, fill with JB, sand flat, repeat application until surface sands smooth.

Tubs can be painted with epoxy paint, but epoxy is very hard to apply. Unless you have means to spray it, best left to the pros.
 
I agree with Steve on having a pro do the painting. I have had two tubs "re finished" and they turned out great. I bet they even have a procedure to repair holes. I have had a few fiberglass tub/showers repaired with fiberglass and you could NOT see the repair when finished. How do I know about fiberglass repair.....had one blow out of the back of my pickup while delivering to a job site. Bad day..
 
I don't remember the name of it,but I got some kind of marine repair putty at the hardware store and fixed mine. It's white and sandable. It's like gas tank repair stuff where you cut off a piece then kneed the outer shell and the inner putty together. It takes some pressure to make it stick down,but like I said,sandable when it sets up. Mine's been in there close to two years now and still holding in good shape.
 
Ya,it hardens like JB Weld,but it's thicker. You definitely have to really push to work it in. The white color is a plus when it comes to bathtub repair.
I put in on some places that were starting to rust but hadn't gone through yet too.
