Bandsaw blade comes off, bad adjuster threads?

Here is what the adjuster threads look like. The threads in the piece it screws into don't look so hot either. In retrospect, it seems like the adjuster started feeling tight when I screwed it up to a certain spot, not just from the blade getting tension and it made a certain squeak. I'll replace the two parts and see what happens. it's pretty hard to get the cell phone to focus right this close up.

Ya seen that more then one time. I picked up a band saw a couple years ago cheap at an auction because the band did not drive with the motor on. Opened up the warm drive area and found a sheared key way replace it and been fine since. Payed something like $20 for it
