Removing stuck large bore adapters

I'm wondering if anyone has any neat tricks for removing the large bore adapters in a rear tube. I've had pretty good luck on average replacing these, I've yet to tear the stem out of a tube, but I've got one that is particularly stubborn. Basically I'm trying to hold the valve stem with thin jawed pliers while unscrewing the adapter with vise grips or the like. If anyone has a foolproof method, I'm all ears. :)
 
Nothing is fool proof.

What I have done to get a better grip is tighten a small hose clamp onto the stem then use a pair of pliers to hold the hose clamp while removing the adaptor.

The clamp also helps prevent the stem from dropping back into the rim if and when the little plastic nut breaks.
