Here are pics of the clutch and worn rivets. Part of the damage was done by my adjustments as there's fresh filings in the bell.Probably a good time to change throw out bearing and rear seal. Both halfs are pointing down to check how bad the seals leak.

Anytime the PTO disc rivets make contact with the back side of the release levers, it's definitely adjustments. Used to see that a lot at the dealer I worked for..
 
When I have to adjust the clutch pedal I like the pto to be disengaged about one inch before the clutch pedal bottoms out on the foot plate so it doesn't push the levers on the pressure plate too far so they won't hit the rivets. On the three adjusting bolts on the three levers no more than two, or three threads of the bolts should be sticking out behind the levers.
