The photos below are of an implement we got with "Sparky" our 51M.

What is the name and/or model of this implement?

It is an M4 tool bar. There are numerous items that you can fasten to the bar. (middle buster, lister, planter, field cultiavtor, and on and on)

I believe it is a M4 or M5 Tool bar with a MA30 Middlebreaker attachment mounted on it. The M4 is 46 inches and the M5 is 68 inches.

I have never seen one of those before except for in the M series sales literature. That would go around my pumpkin and squash hills real nice!
