Case planter

Does anyone know what model of Case planter this is? There is a plate on it that says 764L but I am not sure what it means. I need a planter that can re-seed my hay field without having the field plowed up. Can this planter re-seed my field without having to plow first? Both seed and fertilizer hoppers seem fine, will I be able to fertilize and re-seed at the same time and will the fertilizer hoppers accommodate lime? The tractor I use is a Farmall 560 so I believe there shouldn't be a problem with horsepower. Please and thanks for advice.

Case end wheel drill. Not a no-till drill so field will have to be worked before planting. Lime will not work thru the fertilizer attachment. Put a crescent wrench on the seed shaft to see if it will turn...fertilizer shaft also. Most times after sitting they seize up.

I have use mine with varying degrees of success to "no-till" grass seed into my hay fields, but the ground needs to be really soft and the depth control needs to be at maximum to force the disk into the ground.
