John Deere disc harrow problem

I borrowed this disc from a friend of mine to break up some ground that I had dozed and cleared in preparation for pasture. I'm not familiar with operating farm equipment all that much but I've had the disc bearing block seperate from the frame a couple times (see pictures). I was wondering if this was a common problem? If I need to order parts or what the repair would be...?

My guess is the hole in the brg hanger(yoke) is worn to large allowing brg housing to drop out of hanger. If that is the problem & ""I was lucky enough to choose the correct model"" the part is still available from JD. Q29655 utilizes a 3/4'' bolt to attach hanger to frame.
