Quick hitch with 3 point blade

Have a 4240 with jd quick hitch and 8 ft blade. No problems last year at all moving snow. Last week pushing snow it popped out on one side. The levers were down. On inspection it appears that the gap between the tabs and quick hitch is about the same as the pins.

Is this normal? Or is the blade not designed for the hitch? I don't see how an implement with larger pins would fit.

Are you sure it is not the latches? I picked up a JD category 2 quick hitch pretty cheap because one side would jump out. Disassembled the latch mechanism and cleaned it good, re-adjusted the linkages. But the real problem was that the corner of the latch was rounded off. Welded it back up and re-ground it to profile. The poor adjustment had allowed the implement pins to ride against the corner of the latch and rounded it off. Whole thing is better than new now.
 
Tim, does your blade have these bushings on it?
They help keep the implement from slipping out of
the quick hitch arms, the way you describe.

I don't have those bushings. Sounds like that is what is missing. The latches seem good when the lever on top is pulled up the latch goes flush so they cannot be any longer.

How do those roll pins work on the bushings?
 
Sorry the picture came through 4 times. The roll
pins hold the bushings in place on the lift pins
of your implement. Much the same way as lynch pins
hold a 3-point arm on, if you were not using a
quick hitch.

Roll pins go in flush with the sleeve.

That's what I was thinking you might need.
