JD 165 hydro transmission fluid leak

I notice that the machine wasn't moving correctly. It acted like a tire was spinning and that it would grab and accelerate. Going down hill it would go faster. When I parked it and lifted the seat, I could see that some fluid had leaked out from under the filler cap and after a while the puddle under the machine was about eight inches in diameter. After the machine cooled down, the fluid level was not visible in the tube.

Is it possible that the cap was loose and that all the fluid came out of the cap?
Is it most likely that a seal was let fluid leak out from a shaft, causing the puddle on the concrete?
Why would fluid come out of two places, all at once, when there has never been a problem with it?

The only senario I can think of is not a good one:

Internal leakage due to worn parts in the plunger/rotor group(s) have gotten to the point that the fluid is foaming (picking up air and expanding) and coming through any weak spot it can find in the case. Which would be the cap and the shaft seal in your case.
 
I think I will try to refill it and see if it continues to leak and move erratically.
