Odd head gasket failure

I have a 3550 with the 3 cylinder 183
diesel. Had the engine rebuilt
earlier this year. It started
spitting coolant out the exhaust and
blowing back thru the radiator.
Pulled head off and found the metal
firing ring on #3 cylinder split. Not
burnt out, but split just like
someone cut it open. Smooth even
straight split in firing ring from
cylinder hole into gasket body. Never
seen that before. Anyone else? My
guess is defective gasket. Didn't
see it before assembly.
 
Those engines do not normally fail head gaskets like that, so either it was defective from the start, or maybe damaged during installation.
