6000 s.o.s. B carrier

got my s.o.s. torn down to find the B carrier broke the splines off the inside of the carrier into 3 pieces. I did notice the planetary gears have a lot of movement on the shaft. what would cause this breakage? the movement of the planetary's or someone shifing it reverse to forward without inching pedal? any guesses? don't want it to happen again. only had 100 hrs on it after rebuild.
 
wonder if the endplay on the planetary gears could have caused a wedgeing issue. guess it's possible. the other carrier I have is pretty tight.
 
No telling for sure, worn thrust washers, anything allowing excess play. B-carrier seems to take the brunt of negative forces. Rebuild using used parts usually don't fair well. New parts not available. So no other choice. Once re built a very gentle break-in recommended.
 
The housing is made of cast iron. As you have proably noticed it is kinda thin. If someone would make them in steel it would solve the problem. Machining cost may be prohibitive.
