Commander 6000 engine questions

#1- Should there be copper gaskets on both sides of the
"banjo" (?) bolts on the fuel return line for each injector?
#2- Should there be a triangle shaped hole in the front oil
supply line to the valve train?
Last night I removed the oil pan in preperation for changing
my rear main oil seal. The seal change was pretty
straightforward and went well. As I was getting ready to re
install the oil pan I noticed diesel fuel dripping from #6
lifter. After some inspection I found the diesel coming from
the fuel return line under the valve cover gasket. It ended up
being the injectors were not put back together tight enough
where the top half met the bottom half. Easy fix. Every other
return line I have seen have had copper washers on both
sides of the banjo bolts. This engine only had the washers
on the top side. The manual shows this to be correct but my
local NH shop suggests putting washers from a 5000 on
either side of the banjo bolt. Queestion #1- What do you
think. I tightened it all back down as original and left the
valve cover off to check for oil flow. That is when I noticed
the perfectly shaped triangle hole in the front oil supply line
to the valve train. Question #2- have you ever encountered
a perfectly shaped triangle hole in this line? Sorry for the
long post.
 

I just had the injectors out of my 172. There were two aluminum washers on each bolt. I don't understand the triangular hole in the tube.
