Jim,
I believe that D was used in conjuntion with the daul wheel adapter plate Maybe because it fits the hole better and keeps the plate from damaging the The lug section. Maybe Tony will chime in to confirm this. Photo is on my daul plates with lugs which I purchased all together. I have also seen set that are mixed.

I have a couple of sets of dual wheel spacers/adapter bolts like yours, Kirk.

Though these are not in front of me at this time, I believe that the adapters used with the dual wheel spacers are the same as those used with pie weight sets.

Similar, though slightly longer, adapters without taper on either end are used on row crop (corn picker) wheel spacers.

Hello Kirk , Unfortunately I can't be of much help with the Dual Wheel Spacer kits as I have never had a tractor with a set or seen a set on a low hour tractor that was never altered in some form from stock . It does make sense that they would have the nuts with less hex on them so that they would fit tighter in the spacer and so that it would not wear into the spacer like the sharp edges of the hex would even though they should not be run loose . Have you been working on the your 61 841 4x4 or is it still in the garage just waiting under cover ? Thanks Tony

Hi Tony,
I know what you mean. I've got plenty of projects and a few rolling restorations and still manage to pick up an MF 202, 881 and two 8n's this past week. I don't how my wife puts up with it? I am heading up to Chester Mass to pick up an 8n for a friend in a couple weeks. If we were closer I'd stop by to say hello. It's on the west side and I think it's a little to far out of the way.
