504 pto main drive gear

I have a 504 rowcrop and the pto main drive gear at the front of the tranny keeps coming loose. It doesn"t fit tight and has some sort of a wedge or cone shaped washer that is presses into the gear as you tighten the bolt. IS this the original way it was manufactured or a poor fix by someone? If it is original, how do I keep the bolt tight. I put locktight on the bolt the last time I tightened it and it lasted two seasons of haying but now sounds like it is loose again. PLEASE give some advise.
 
Followup, I just looked on the caseih site and the diagram does show a gear wedge. So I guess its a factory design but it leaves the gear with so much slack that I don't understand how it can possibly stay tight.
 
Make sure the flat washer that retains the wedge is not bent to a cone shape. The later tractors came with a heavier washer.
 
Notice the early serial number tractors did not use a wedge. I believe on those the shaft was longer and came closer to going through the gear or the gear was slightly different. Some one may have done some changing over the years to try to keep that gear tight. I never saw one get loose that used the wedge. The older ones came loose and that is why they went to the wedge.
