Farmall M Pops out of 4th

I am refurbishing this tractor for a senior project in high school. It is a '41
M. When I was using before I started tearing it down I noticed it popped out of 4th occasionally. Only 4th though. I have only taken the cover off the transmission,as of yet. Before I start tearing into it I was wondering what would be the most likely cause?

Center pilot bearing between the input and output shafts could be coming apart. The gears could be worn and tapered thus pushing it out of engagement.
The shifter fork could be bent, not putting it all the way in.
The shift rail could have rusted or missing detent spring and ball that position the fork and gear.
