What caused this and how can i keep from happening again

I have to face this every day, I am an internet junky. I read but try to stay on the sidelines now! But I need some insight and guidance here.
4490 major out of frame about 700 hrs. Used oil right out of the box just under a quart an hour, have tried numerous remedies last was some Bon-ami. Tore down here was what I see. This slick hole is number 4 very little cross hatch left. 3 right beside looks good, as do the other five.
(Federal mogul FP-57set) from reputable local supplier. Top ring looks good as does the oil ring. Middle ring worn all the way across and sharp. Thanks Tom

What was the piston clearance? Ring gap on all rings? Ring clearance on all rings? Sleeve out of round? Piston out of round? Wrist pin fit? Piston clearance to head compared to others?
 
If check injector on all cylinders but especially number if it is
not up to specs it may be washing down that cylinder. Causing
your problem. We had bio diesel get our 2590 and had to run it
with a bit of miss to get through spring and it grotto the point it
was using 6 quarts of oil a day by the time we finished and the
cylinders looked similar to yours.
 
A bent con rod will also cause this type of wear.
A hydrauliced cylinder due to bad injector (ala newer Fords with the electronic injectors) will sure cause it,if you're lucky. I've also seen cylinder blocks with a bore off (factory). Certainly not common. Had an older 454 Chev block out of a 1 ton that required a piston, just like clockwork every 30,000 or so. First couple under warranty and then he's on his own. Found one cylinder .060 out of true with the deck and crank center line. Just one, the tooling was at the end of it's cycle I guess

Liner flange looks very pitted,other one does
not. Also hole in deck opposite side # 2 photo,
looks quite black around it. Getting water some
where and leaking comp in that water hole.

Comp rings have little or no effect on oil
usage unless they make crankcase pressure that is
returned to intake and pushing oil with it. Oil
rings have all the oil control of oil.

Is it natural or turbo ,turbo could be the
oil problem . If natural the water may have
come down the stack.

Last 504 I rebuilt (1450 B loader)I used
clevite kits. I use FP parts when I have to ,
but prefer clevite.Helps to be a clevite
distributor.

Agree with George, looks like you have something going on around the water passage hole. Was it losing water also? Antifreeze can cause lots of problems when it gets into the cylinder.
