A early Case CC two hole goose. Seldom seen.

I only found one poor picture on the net. I think one of the Case books shows that front end pretty well. There was a gear reduction on the steering that hooked on the hole. I think only the very early '30 (or 29) models had that. I have seen several with the added on cultivator mounts, but none had the extra hole for that style of steering.
 
There was an early CC at the Baraboo WI show this year with this style gooseneck on it. It had the later style steering arm, not the geared one.
 
Nedd, this might help you picture it. The second hole is where the cast iron piece pivots. The inside of the fan shape has am arced rack shape built in that meshes with a pinion gear on top of the steering post. As the steering rod end of the cast fan get pushed or pulled, the casting pivots on the pin beside the steering post, sweeping the rack teeth and thus spinning the steering post gear. DKase mentioned that there were actually three versions of the cast fan. I think one had the ends open, the next had the ends on the fan closed, and the third had more reinforcing ribs cast in.

I can't see one on the goose, must be behind the place where it bolts on. On the bolster it is 4591A.
