Can anyone help me identify this planter? Model number, production years, can it be converted to a snap coupler? Also any idea what it's worth? Any help would be appreciated.

I have the exact same planter. Not sure about converting it to snap coupler but if you have a drawbar, it attaches to that. The pin goes through your pin hole and a bolt goes through the front of the mount on the planter, and the bolt will go through the slide where your drawbar hangar is. The drawbar needs to be set to its shortest length. I am not sure about value but the fertilizer bins are not correct. They are for a newer planter. The originals were either straight or funnel shaped, depending on the model. The originals had a tendency to rust out where they bolted to the base. I have seen these planters sold at auctions. Cheapest i have seen was $25, in worse condition than yours, the highest was $400, completely restored.
 
Planter is called a "Drill Planter" I recently bought one very similar to yours. Mine is a snap coupler model. I know they were made in the late 50's with metal fertilizer canisters. In the early 60's they switched to the fiberglass fertilizer canisters. Looks like you picked up a very nice one!
